Description

Cho et al identified factors associated with flares in Asian patients with systemic lupus erythematosus (SLE). These can help to identify a patient who may benefit from more aggressive management. The authors are from National University Hospital of Singapore.

Patient selection: Southeast Asian patient with stable systemic lupus erythematosus

Outcome: flare (defined by SLEDAI-2K, with stable disease SLEDAI-2K <=4)

Predictors for a flare:

(1) younger age (lowest tertile < 40 years of age)

(2) longer duration of disease (duration > 12 years)

(3) higher baseline prednisolone dose (suggesting more active disease)

(4) thrombocytopenia (< 100,000 per microliter)

(5) hypocomplementemia (C3 < 85 mg/dL; C4 < 10 mg/dL)
