Chan et al identified risk factors for left ventricular dysfunction in an asymptomatic patient with mitral valve regurgitation (MVR). This can help to identify a patient who may benefit from surgery to repair the valve. The authors are from the University of Ottawa in Canada.
Patient selection: asymptomatic adults with severe mitral regurgitation associated with myxomatous degeneration
Outcome: left ventricular dysfunction (LV ejection fraction < 60%)
Predictors of left ventricular dysfunction:
(1) indexed left ventricular end-systolic diameter (ILVESD) > 19 mm per square meter BSA
(2) preoperative right ventricular systolic pressure > 45 mm Hg
A patient with one or both predictors should be considered for mitral valve repair.
