Description

Chamnanchanunt et al identified findings predictive of significant clinical bleeding in adults with dengue fever. These can help to identify patients who may require more aggressive management. The authors are from Mahidol University, Sri Ramachandra University and Chulalongkorn University in Thailand and India.


Patient selection: adult with dengue fever

 

Outcome: clinically significant bleeding (gastrointestinal bleeding, hematuria, menorrhagia = Chhina Type 1)

 

Predictors from multivariate analysis:

(1) female gender

(2) severe thrombocytopenia on admission (platelet count less than 25 * 10^9/L)

(3) absolute lymphocyte count > 550 per µL (0.5 * 10^9/L)

 

Additional findings from univariate analysis:

(1) vomiting (a finding associated with severe dengue fever)

(2) high serum AST (> 200 IU/L with upper limit of normal < 40 IU/L)
