Additional factors for >= 80 years:
(1) history of hospital admission less than 3 months age
(2) respiratory tract infection
(3) systolic blood pressure <= 100 mm Hg
(4) low oxygen saturation (not specified, possibly < 94)
Additional factors for 65 to 79 years:
(1) body temperature (not specified, presumably hypothermia since temperature range 36.8 to 37.9°C in Table S1)
