Description

Blondal et al identified risk factors for recurrence of multidrug-resistant or extensively drug-resistant tuberculosis. These can help to identify a patient who may benefit from more aggressive management. The authors are from Tartu University, Reykjavik Health Care Services and the Estonian Tuberculosis Registry.

Patient selection: multidrug-resistant or extensively drug-resistant tuberculosis in Estonia

Risk factors for recurrence of the tuberculosis:

(1) resistance to all injectables

(2) resistance to a greater number of drugs

(3) sputum smear positivity at start of therapy

(4) male sex

(5) birth outside of Estonia

(6) poor adherence

For a patient who has been successfully treated a history of previous therapy for tuberculosis is associated with increased risk of recurrence.

Therapy with as many anti-tuberculosis drugs as possible was associated with a reduced risk for recurrence.
