Bassetti et al reported predictors of septic shock in a patient with candidemia. This can help to identify a patient who may require more aggressive management. The authors are from multiple institutions in Italy and Spain.
Patient selection: candidemia
Predictors of septic shock from multivariate analysis:
(1) age > 50 years (OR 2.6)
(2) intra-abdominal source (OR 2.2)
(3) admission to general ward at the time of candidemia onset (OR 0.21; ? indicative of stable clinical status)
Predictors of 30-day mortality in a patient with candidemia:
(1) septic shock attributable to candidemia (OR 2.1
(2) higher Charlson comorbidity index (OR 1.3)
(3) hemodialysis after candidemia onset (OR 8.6)
(4) intra-abdominal source (OR 1.9)
(5) central venous catheter source (OR 0.4)
