Description

Hassan and Rehim used the iMELD score to predict in-hospital mortality for a patient with cirrhosis and spontaneous bacterial peritonitis. This can help to identify a patient who may benefit from more aggressive management. The authors are from Assiut University in Egypt.


Patient selection: cirrhosis with spontaneous bacterial pneumonia

 

Outcome: in-hospital mortality

 

Risk factors for in-hospital mortality:

(1) older age

(2) high serum creatinine

(3) high serum total bilirubin

(4) low serum sodium

 

Parameters for the iMELD:

(1) MELD (modified, which includes serum creatinine and serum total bilirubin)

(2) age in years

(3) sodium in mmol/L

 

iMELD =

= (MELD) + (0.3 * ((age)) - (0.7 * (sodium)) + 100

 

Interpretation:

An iMELD score > 43.5 is associated with in-hospital mortality.

 

Performance:

The area under the ROC curve is 0.86.
