The PredAHT (Predicting Abuse Head Trauma) is a clinical prediction tool that can aid in the diagnosis of abusive head trauma.
Patient selection: age < 36 months with intracranial injury
Parameters:
(1) rib fractures
(2) retinal hemorrhages
(3) apnea
(4) long bone fractures
(5) head and neck bruising
(6) seizures
If >=3 of the findings are present, then the OR for abuse head trauma is > 100.
If (rib fracture or retinal hemorrhage) is present AND if >=1 of the other findings is present, then the OR for abuse head trauma is > 100.
The OR for abusive head trauma is > 10 when:
(1) rib fracture is present by itself
(2) retinal hemorrhage is present by itself
(3) long bone fracture is present by itself
(4) 2 of the findings are present
If apnea alone is present, then the OR for abuse head trauma is 6.9.
