Description

Chao et al identified factors associated with a poor prognosis for pediatric patients with pneumococcal meningitis. These can help to identify patients who may benefit from more aggressive management. The authors are from Mackay Memorial Hospital in Taipei.


Patient selection: pediatric patient with pneumococcal meningitis

 

Predictors of mortality:

(1) altered mental status on admission

(2) shock on admission

(3) endotracheal tube intubation on admission

(4) hyponatremia on admission (serum sodium < 130 mmol/L)

(5) CSF white blood cell count < 20 per microliter

(6) low CSF glucose concentration

(7) elevated ratio of CSF to blood glucose concentration

 

Predictors of neurologic sequelae during the hospitalization:

(1) seizures

(2) focal neurological signs
