Maternal uteroplacental vascular compromise (UPVC) can be diagnosed if the placenta shows certain pathologic changes.
Criteria for UPVC - one or both of the following:
(1) placental infarction (s)
(2) histologic score
Placental infarcts indicative of UPVC - at least one infarct that is:
(1) > 2 cm from the edge of the placenta (nonmarginal)
(2) volume > 1 cubic cm (spherical infarct with a diameter > 1.25 cm)
Parameters for histologic score:
(1) syncytial knotting
(2) syncytial basophilia
(3) villous fibrosis
(4) excess perivillous fibrin deposition with cytotrophoblast proliferation

Interpretation:
• minimum histologic score: 0
• maximum histologic score: 16
• A score >=8 was indicative of UPVC.
