Description

A patient with hemophilia can develop hematomas at many sites, such as the upper airway.


Hematomas may be present in any combination of pharynx, epiglottis, or larynx.

 

Triggers:

(1) pharyngitis or laryngitis

(2) trauma

(3) iatrogenic

 

Clinical features:

(1) neck ecchymosis

(2) variable neck hematoma

(3) throat discomfort

(4) variable dyspnea

 

Large hematomas may cause respiratory distress.

 

The hematomas often resolve after therapy with recombinant factor 8. Percutaneous drainage can be attempted if the hematoma is large and if a safe path can be visualized.
