Description

Liu et al reported the peri-operative transfusion trigger score of emergency (POTTS-E) for evaluating appropriateness of blood transfusion in a patient undergoing emergency surgery. The authors are from Affiliated Hospital of Zunyi Medical College and Sichuan University in China.

Patient selection: emergency surgical patient with hemoglobin < 10 g/dL, age 44 to 55 years of age

Exclusion: cancer, hematologic disorder, history of coronary artery disease, obstetrical patient, nerve surgery

Parameters:

(1) shock index

(2) nasopharygeal temperature in °C

(3) FIO2 in percent (from 21 to 100%) required to keep >= 95% on pulse oximetry

(4) epinephrine in micrograms per kg per minute

total score =

= SUM(points for all 4 parameters) + 7

Interpretation:

• minimum total score: 7

• maximum total score: 15

• Blood transfusion not needed if hemoglobin > 10 g/dL or total score 7.

• For a score between 7 and 10 then the total score could be used as a target for blood transfusion.
