Description

The peri-appendiceal red patch (PARP) can be seen in some patients with ulcerative colitis (UC).

Synonym: appendiceal orifice inflammation (AOI)

Features of the peri-appendiceal red patch:

(1) The patient has ulcerative colitis involving the distal colon.

(2) There is inflammation at the appendiceal orifice and adjacent cecum.

(3) Normal colon separates the distal colon and cecum, consistent with a “skip” lesion.

(4) The inflammation in the distal colon and cecum show comparable histologic activity.

There tends to be a male predominance in this pattern of involvement.
