Description

Rarely bone may develop as a metaplastic process in the penis. The affected patient can really say that he has a boner.

Presentation: hard mass in the penile shaft, which may be painful or painless

Risk factors:

(1) dystrophic calcification of the penis (secondary to end-stage renal disease, hypercalcemia, etc)

(2) trauma

(3) penile implant

(4) Peyronie plaque

Histologic exam of the material shows cortical bone. A larger piece may show a medullary space with marrow elements.

It is important to recognize the process prior to surgery since surgical reconstructions can be challenging and there is an increased risk for complications.

There should be no other diagnosis that can explain findings better (teratoma, extramedullary hematopoiesis, etc).
