Description

Brucellosis may be associated with a reversible pancytopenia. The pancytopenia can be due to several mechanisms, especially hypersplenism.


Clinical features:

(1) The pancytopenia tends to be reported more often in children.

(2) The patient typically has other features of infection with fever, malaise, weight loss and hepatosplenomegaly.

(3) Blood cultures are often positive.

 

Hematologic findings:

(1) The patient has anemia, thrombocytopenia and leukopenia, which can be severe.

(2) The bone marrow may be hypercellular, normocellular or hypocellular.

(3) Non-caseating granulomas may be seen.

(4) Hemophagocytosis is common.

(5) Histiocytes, plasma cells and eosinophils may be increased in the aspirate.

 

The pancytopenia reverses on therapy if brucellosis is the sole process.
