Pancreaticobiliary maljunction is an uncommon malformation of the pancreaticobiliary tract.
Anatomic features:
(1) convergence of the pancreatic and bile ducts outside of the duodenum wall
(2) abnormally long common channel (> 5 mm)
Diagnosis is usually established using imaging studies:
(1) ERCP, MRCP or CT
(2) intraoperative cholangiography
(3) ultrasonography
It may be associated with congenital choledochal cyst.
The condition typically presents during childhood.
Presentations may include:
(1) jaundice
(2) vomiting
(3) abdominal pain
(4) abdominal mass
(5) fever
Complications:
(1) chronic cholangitis
(2) ascites
(3) protein plus in the pancreatic duct
(4) pancreatitis
(5) biliary rupture
(6) cholangiocarcinoma (high risk)
