Description

Orthostatic or postural proteinuria refers to mild proteinuria associated with an upright posture that remits upon recumbency. It is a benign condition that needs to be distinguished from more significant renal disease.


 

Features:

(1) Most patients are children or young adults.

(2) The proteinuria is present in samples collected during the day.

(3) The proteinuria is absent on first morning urine specimens collected immediately upon arising (the testing should be performed on at least 2 occasions)

(4) The amount of urine protein is usually small (rarely > 1 gram per 24 hours).

(5) There are no other conditions that can explain the proteinuria better.

 

Hopefully a renal biopsy would not be performed on the patient. If one is done then there is no or minimal renal pathology present.
