Description

Thomassin-Naggara et al reported a magnetic resonance imaging grading system for sonographically indeterminate adnexal masses.

Patient selection: indeterminate mass on ultrasound

Analysis: MRI

Finding

Grade

no adnexal mass

1

pelvic mass not adnexal

1 (grade separately)

purely cystic mass

2

purely endometriotic mass

2

purely fatty mass

2

no wall enhancement

2

solid tissue with homogeneous hypointense T2-weighted AND homogeneous hypointense high b value of diffusion-weighted solid tissue

2

unilocular cyst with proteinaceous or hemorrhagic fluid AND does not comply with endometriotic fluid signal intensity AND no solid tissue

3

multilocular cyst and no solid tissue

3

mass with solid tissue that is enhanced with time intensity curve type 1 on dynamic contrast enhanced MRI (excluding dark-dark solid tissue)

3

adnexal mass with solid tissue that enhanced with a time intensity curve type 2 on dynamic contrast enhanced MRI (excluding dark-dark solid tissue)

4

solid mass with solid tissue that is enhanced with a time intensity curve type 3 on dynamic contrast enhanced MRI

5

omental thickening or nodules detected
