Description

Shoobridge et al reported a nomogram for predicting surgical exploration in a patient with renal trauma. This is a modification of the nomogram of Shariat et al. The authors are from Alfred Hospital, Royal Melbourne Hospital, University of Melbourne and Guy's and St Thomas' NHS Foundation Trust.

Patient selection: trauma

Parameters:

(1) renal injury grade

(2) number of units of platelet transfused in the first 24 hours

(3) serum BUN in mg/dL

(4) hemoglobin in g/L

points for serum BUN =

= MIN(33.3,0.555*(serum BUN))

points for platelets =

= MIN(57.4,3.5875*(number of platelet products))

points for hemoglobin =

= MAX(0,0.064107 * (hemoglobin)-4.4946)

total score =

= SUM(points for all 4 parameters)

Interpretation:

• minimum score: 0

• maximum score: 198.4

• The higher the score the greater the risk of surgical exploration and Nephrectomy.
