Description

Demir et al reported the NIKEI score for predicting advanced fibrosis in patients with non-alcoholic fatty liver disease (NAFLD). This can help reduce invasive studies needed to exclude liver fibrosis. The authors are from University Hospital of Cologne and University Hospital of Essen.

NIKEI is an acronym for Non-Invasive Koln-Essen Index.

Patient selection: non-alcoholic fatty liver disease

Parameters:

(1) age in years

(2) serum AST in U/L

(3) serum ALT in U/L

(4) serum total bilirubin in mg/dL

AST to ALT ratio =

= (serum AST) / (serum ALT)

Parameter

Points

age in years

0.225 * (age)

serum AST

0.056 * (AST)

AST to ALT ratio

5.044 * (ratio)

serum total bilirubin

3.631 * (bilirubin)

X =

= SUM(points for all 4 parameters) - 24.214

probability of advanced fibrosis =

= 1 / (1 + EXP((-1) * X))

A probability >= 0.2294 defined presence of advanced fibrosis.

A probability <= 0.0535 defined absence of advanced fibrosis.

A probability from 0.0536 to 0.2293 was indeterminate.

Performance:

• The area under the ROC curve is 0.97.
