Both Nerot et al and Sirveaux et al reported a similar classification for scapular notching following total shoulder arthroplasty for massive rotator cuff tear.
Scapular notching is a defect in the bone along the inferior part of the glenoid component of a total shoulder arthroplasty.
Bone Defect
Grade
involves only the inferior pillar of the scapular neck (most shallow)
1
reaches the lower screw (lateral end)
2
extends over the lower screw (medial end)
3
extends beyond the baseplate
4
Grades 1 and 2 are caused by mechanical impingement between the humeral polyethylene component and glenoid rim.
Grades 3 and 4 are caused by polyethylene-induced osteolysis and may require revision arthroplasty if erosion affects glenosphere fixation or causes instability
