where:
• The level of evidence does not cover low or no levels of evidence. Presumably that should be Category 3 despite the criteria for Category 2.
• The scheme does not cover high level of evidence with less than uniform consensus. In the implementation this will be Category 2a.
Categories 1 and 2A are usually viewed as good evidence for an action.
