A small percentage of patients who receive an mRNA-based COVID-19 vaccine may develop a myocarditis.
Typical patient: pediatric patient or young adult, more often male
Onset: often within days of vaccination, but can extend to 6 months
Clinical findings:
(1) chest pain or discomfort
(2) palpitations
(3) dyspnea
(4) syncope
Additional findings:
(1) elevated serum troponin
(2) abnormal ECG
(3) abnormal echocardiogram
(4) abnormal cardiac MRI
Other diagnoses and other causes of myocarditis need to be excluded.
Most cases are mild and resolve with conservative therapy.
