Description

The morphology of myeloma at the time of diagnosis can be used to predict patient survival.

Morphologic categories of plasma cells in the bone marrow:

(1) mature plasma cells

(2) proplasmablasts

(3) plasmablasts

(4) lymphoplasmacytoid cells

Morphology

Content

mature

more than 50% of plasma cells are mature

proplasmablastic

more than 50% of plasma cells are proplasmacytic

plasmablastic

more than 50% of plasma cells are plasmablasts

mixed

no category of plasma cell was more than 50%

Since both the proplasmablastic and mixed types showed a similar prognosis, they were combined into an "immature" group.

Morphology

Prognosis

mature

best

immature (proplasmablastic and plasmablastic)

intermediate

plasmablastic

worst

The presence of renal insufficiency also adversely affects survival.

estimated survival in months =

= 69.0 - (51.45 * (1 if plasmablastic type, else 0)) - (35.03 * (1 if immature subtype, else 0)) - (16.4 * (1 if renal insufficiency present, else 0))
