Description

Patients with myeloma who have a high LDH concentration at presentation have a worse prognosis.

An elevated serum LDH in a patient with myeloma may be associated with:

(1) a rapidly progressive course

(2) a shortened time to relapse following remission

(3) decreased survival time

(4) poor response to chemotherapy (nonresponders)

(5) more extensive bone marrow infiltration

Limitations:

• The cutoff for the serum concentration depends on the assay being used.

• The assay should be standardized.

• An elevated LDH may be due to other cause (hepatitis, hemolytic anemia, etc).
