Description

Hill et al developed an index for evaluating renal biopsies from a patient with lupus nephritis. One component is the glomerular activity index (GAI). The authors are from Hopitaux Broussais, St. Louis, Henri Mondor and Bichat in Paris.


Patient selection: lupus nephritis

 

Specimen: renal biopsy

 

Parameters:

(1) glomerular proliferation (glprolif)

(2) polymorphonuclear leukocytes (glpmn)

(3) karyorrhexis/fibrinoid necrosis (karyfib)

(4) cellular cresecents (cresc)

(5) hyaline deposits (hyaldep)

(6) glomerular monocytes (glmono)

 

Each parameter is scored according to the severity of the finding.

glomerular activity index =

= (points for glomerular proliferation) + (points for polymorphonuclear leukocytes) + (2 * (points for necrosis)) + (2 * (points for cellular crescents)) + (points for hyaline deposits + (points for glomerular monocytes)

 

Interpretation:

• minimum score: 0

• maximum score: 24

• The higher the score the greater the glomerular involvement.
