At the 2005 Montreal World Congress of Gatroenterology a classification for ulcerative colitis (UC) was proposed to describe the extent and severity of disease involvement. This can improve communication about a patient affected by the disease.
Parameters:
(1) extent (E)
(2) severity (S)
Extent |
Term |
Code |
limited to the rectum (distal to rectosigmoid junction) |
ulcerative proctitis |
E1 |
limited to colon distal to the splenic flexure |
distal (left-sided) UC |
E2 |
involvement of colon proximal to the splenic flexure as well as distal |
extensive UC (pancolitis) |
E3 |
Severity |
Term |
Code |
clinically asymptomatic |
clinical remission |
S0 |
>= 4 stools per day with or without blood; no systemic illness; normal ESR and other inflammatory markers |
mild disease |
S1 |
>= 5 stools per day with or without blood; minimal signs of systemic illness |
moderate disease |
S2 |
>= 6 stools with blood per day; signs of systemic illness |
severe disease |
S3 |
Signs of systemic illness for S3 include:
(1) pulse >= 90 beats per minute
(2) temperature >= 37.5°C
(3) anemia (hemoglobin < 10.5 g/dL)
(4) ESR >= 30 mm in first hour
