Description

A patient who receives a monoclonal antibody as targeted therapy may show a transient monoclonal band in serum protein electrophoresis (SPE).

Patient selection: monoclonal band in serum protein electrophoresis

Clinical features:

(1) history of parenteral administration of a monoclonal targeted therapy in the past 3 months

(2) a dose of targeted therapy sufficient to produce the size of the monoclonal band

Confirmation:

(1) serum concentration of the monoclonal antibody matching the size of the monoclonal band

(2) immunoreactivity based on the animal species involved

(3) electrophoretic mobility of the monoclonal band matches that antibody class of the therapeutic

Problems in diagnosis:

(1) patient with monoclonal gammopathy due to myeloma

(2) lack of information about monoclonal antibody therapy
