Description

A patient receiving an oral anticoagulant should be monitored for warfarin-related nephropathy (anticoagulation-related nephropathy) during therapy.


Testing periods are separated into the initiation phase (first 3 months) and maintenance phase (thereafter).

 

Since chronic kidney disease is a risk factor for anticoagulation-related nephropathy, testing is also affected by the patient's GFR in mL per minute.

 

Additional instructions:

(1) Rapid changes in the INR should be avoided.

(2) The patient should report any hematuria.

 

Testing protocol:

(1) serum creatinine in mg/dL

(2) INR

 

Testing during the initiation period (first 3 months) every 3-4 weeks.

 

Testing during the maintenance phase is determined by the anticoagulant and GFR.
