The modified SOAR (mSOAR) scale can help to predict patient mortality associated with an acute stroke.
SOAR is an acronym for Stroke type, Oxfordshire Community Stroke Project classification, Age and prestroke modified Rankin status.
The modification is the addition of the admission NIHSS score
Patient selection: acute stroke
Outcome: in-hospital, 7-day mortality and discharge disability
Parameters:
(1) age in years
(2) stroke type
(3) Oxfordshire Community Stroke Project (OCSP) classification
(4) prestroke modified Rankin
(5) admission NIHSS score

= SUM(points for all 5 parameters)
Interpretation:
• minimum score: 0
• maximum score: 9
• The higher the score the greater the discharge disability (modified Rankin scale).
Performance:
• The area under the ROC curve for moderate to severe disability is 0.86.
• The area under the ROC curve for inpatient mortality is 0.97.
