Karam et al reported a frailty index for use in patients undergoing vascular surgery. The authors are from Henry Ford Hospital and the University of South Florida.

Patient selection: vascular surgery

Parameters:

(1) diabetes

(2) not functionally independent

(3) chronic obstructive pulmonary disease (COPD) or pneumonia

(4) congestive heart failure

(5) history of myocardial infarction

(6) one of: PCI, CABG or angina

(7) hypertension requiring medication

(8) peripheral vascular disease or rest pain

(9) impaired sensorium

(10) history of transient ischemic attack or transient cerebrovascular accident

(11) history of cerebrovascular accident with neurologic deficit

Response

Points

absent

0

present

1

frailty index =

= SUM(points for all 11 parameters) / 11

Interpretation:

• minimum frailty index: 0

• maximum frailty index: 1.0

• The higher the index the greater the frailty, surgical site infections and the risk of mortality.
