Parameters:

(1) history of diabetes mellitus

(2) impaired functional status

(3) history of COPD or pneumonia

(4) history of congestive heart failure

(5) history of myocardial infarction within the past 6 months

(6) history of percutaneous coronary intervention (PCI)

(7) cardiac surgery or angina

(8) therapy with antihypertensive medications

(9) peripheral vascular disease or rest pain

(10) impaired sensorium

(11) history of transient ischemic attack or cerebrovascular accident with persistent residual defect
