Modified ATS Rule of Ewig et al for Identifying a Patient with Severe Community-Acquired Pneumonia

 

Criteria - at least one of the following:

(1) one or both of the following major criteria on admission:

(1a) requirement for mechanical ventilation

(1b) septic shock

(2) two or three of the following minor criteria on admission:

(2a) systolic blood pressure < 90 mm Hg

(2b) multilobar (3+) involvement

(2c) PaO2 to FIO2 ratio < 250

 

Performance:

• The sensitivity was 78% and specificity 94%.

• The positive predictive value was 75% and negative predictive value 95%.
