Description

DiGiorgio et al modified the National Health and Safety Network (NHSN) definition of central line-associated bloodstream infection (CLABSI) in patients with hematologic malignancy. This is meant to avoid false positive diagnosis caused by a mechanism other than the catheter. The authors are from the Cleveland Clinic.


Premise: in a patient with a hematologic malignancy the presence of treatment-related mucositis, graft-versus-host disease (GVHD) or neutropenia makes the patient susceptible to certain infections independent of a central catheter.

 

Patient selection: patient who meets the NHSN definition of CLABSI

 

The modification is to not call the situation CLABSI in any of the following situations:

 

Infectious Agent(s)

Situation

viridans group Streptococcus

mucositis

Enterococcus, Enterobacteriaceae or Candida species

bone marrow transplant recipient with graft-versus-host disease of the GI tract

Enterococcus, Enterobacteriaceae or Candida species

neutropenia due to dose-intensive chemotherapy
