Description

Shackelford et al used point of care laboratory measures to predict the likelihood of blood transfusion in  a trauma patient. The authors are from the University of Maryland, the USAF Center for Sustainment of Trauma and Readiness Skills and te ONPOINT Study Group.

Patient selection: trauma

Point of care analyzer: iStat

Outcome: transfusion within 3 hours

Parameters (Table 3):

(1) serum bicarbonate in mmol/L

(2) hematocrit as a decimal fraction

(3) blood glucose in mg/dL

(4) INR

(5) serum lactate in mmol/L

Parameter

Points

bicarbonate

-0.2867 * (bicarbonate)

hematocrit

-0.5438 * (hematocrit)

blood glucose

0.43 * (glucose)

INR

0.1471 * (INR)

serum lactate

0.3255 * (lactate)

where:

• The units were not specified in the paper. The ones selected are based on trial and error.

X =

= SUM(points for the 5 parameters) - 3.2995

probability of packed red blood cells within 3 hours =

= 1 / (1 + EXP((-1) * X))
