Description

Boutault et al developed a model based on changes in the complete blood count to identify a patient with unexplained cytopenia who may have a myelodysplastic syndrome (MDS). The authors are from CHU Nantes, Avicenne University Hospital, Technischen Universitat Dresden and University Hospital Leipzig

Patient selection: unexplained cytopenia

Parameters:

(1) MCV in femtoliters (tended to be higher in MCV)

(2) Ne-WX (neutrophil complexity and width of dispersion of the events measured = coefficient of variation of the light scatter around neutrophils = neutrophil dispersion relative to granularity): available on the SYSMEX XN-10 analyzer. Scores in controls ranged from 223 to 476.

(3) absolute neutrophil count (ANC) in 10^9/L

X =

= (0.10285 * (MCV)) - (0.38466 * (ANC)) + 0.03294 * (Ne-WX)) - 21.02807

MDS-CBC score = probability of MDS =

= 1/(1+EXP((-1) *X))

An MDS-CBC score > 0.20 should be evaluated further for MDS.

Performance:

The area under the ROC curve was 0.92.
