Description

Ben Ayed et al reported a simple score for identifying a patient with a community-acquired urinary tract infection (CUTI) caused by multidrug-resistant Enterobacteriaceae. This can help to identify a patient who may benefit from more aggressive management. The authors are from Hedi Chaker University Hospital and University of Sfax in Tunisia.

Patient selection: community-acquired urinary tract infection

Parameters:

(1) age in years

(2) diabetes

(3) history of urinary tract surgery in the past 12 months

(4) history of antimicrobial therapy in the past 3 months

Interpretation:

minimum score: 0

maximum score: 18

A score >= 2 (seen if any of the risk factors is present) is associated with MDR Enterobacteriaceae.

Performance:

The area under the ROC curve is 0.71.

A score >= 2 has a sensitivity of 71% and specificity of 60%.

A score >= 6 has a sensitivity of 43% and specificity of 85%.
