Description

Following an industrial accident or explosion the emphasis is placed on managing the traumatic injuries. If the accident or explosion involves a barium salt, then barium intoxication may complicate the problems.


 

Barriers to recognition:

(1) failure to obtain history of chemical exposures

(2) failure to consider the importance of the barium salt exposure

(3) coma or sedation that prevent recognition of abdominal pain and muscle weakness

 

Keys to recognition:

(1) profound hypokalemia

(2) excessive salivation

(3) flaccid paralysis or decreased deep tendon reflexes

(4) ECG changes not explained by traumatic injuries

(5) unexplained pulmonary edema or respiratory failure

 

Measurement of barium concentration in blood or urine can establish or exclude the diagnosis.
