Antisynthetase syndrome (anti-Jo-1 syndrome) may be misdiagnosed as rheumatoid arthritis, especially early in its course.
The classic triad for antisynthetase syndrome:
(1) polyarthritis
(2) myositis
(3) interstitial lung disease
Most patients present without the complete triad, often with arthritis alone and with positive rheumatoid factor.
Misdiagnosis can be avoided by:
(1) testing for antisynthetase antibodies (anti-Jo-1, anti-Ro-52, anti-PL-7, anti-PL-12)
(2) diagnostic review with appearance of myositis, interstitial lung disease, Raynaud's phenomenon, skin rash or mechanic's hands
