Description

Yamamoto et al listed criteria for the diagnosis of Mikulicz's disease. These criteria can help to distinguish these patients from those with other conditions such as Sjogren's syndrome. The authors are from Sapporo Medical University in Hokkaido, Japan.


 

Criteria for the diagnosis of Mikulicz's disease - all of the following:

(1) bilateral, symmetrical and persistent swelling involving more than 2 lacrimal and major salivary glands (parotid or submandibular glands)

(2) prominent mononuclear infiltrate of the affected glands

(3) exclusion of other conditions causing enlargement of lacrimal and salivary glands (sarcoidosis, lymphoproliferative disorder, viral infection, etc.)

 

Many cases are part of the IgG4-related sclerosing disease with:

(1) elevated serum IgG4 concentrations

(2) a large number of IgG4-positive plasma cells in the inflammatory cell infiltrate
