Description

Huo et al reported the MESO index for evaluating a patient with hepatic cirrhosis. This can help to identify a patient with a poor prognosis. The authors are from National Yang-Ming University and Taipei Veterans General Hospital in Taiwan.

Patient selection: hepatic cirrhosis

Outcome: 3-month mortality

Parameters:

(1) modified MELD score (original x10)

(2) serum sodium concentration in mmol/L (mEq/L)

MESO index =

= (MELD) / (serum sodium) * 10

Interpretation:

• The higher the MESO index the worse the survival.

Parameter

Risk Group

50% Mortality

< 0.8

low

85 months

0.8 to 1.6

intermediate

30.3 months

> 1.6

high

1.5 months

Performance:

• A MESO index > 1.6 is 50% sensitive and 97% specific for 3-month mortality.

Other predictors of 3-month mortality:

(1) modified MELD score > 12 (86% sensitive, 52% specific)

(2) serum sodium concentration <= 136 mmol/L (86% sensitive, 77% specific)
