Description

Peth reported possible causes of medication errors in the Emergency Department. These are divided along the stages of the medication process, including drug administration. The authors are from the University of Missouri.


Errors that may impact drug administration:

(1) carrying out an order that is questionable (illegible, dangerous, confusing, etc)

(2) failure to conduct double checks prior to administration

(3) administration to the wrong patient

(4) administration of the wrong drug

(5) administration of the wrong dose

(6) administration by the wrong route

(7) co-administration of 2 or more drugs that are incompatible

(8) problems with intravenous or enteral access

(9) barriers to report or respond to administration errors
