Description

Welch et al reported echocardiographic criteria for the diagnosis of constrictive pericarditis. The authors are from the Mayo Clinic.


Assessment: echocardiography

 

Hepatic vein velocities measured by Doppler in the subcostal window:

(1) forward velocity in systole and reversal velocities in diastole during inspiration

(2) forward velocity in systole and reversal velocities in diastole during expiration

 

hepatic vein expiratory diastolic reversal ratio in expiration =

= (diastolic reversal velocity in expiration) / (forward velocity in expiration)

 

Echocardiographic findings associated with constrictive pericarditis:

(1) ventricular septal shift (VSD; any degree of cyclic movement of the ventricular septum toward the left ventricle with inspiration and toward the right ventricle with expiration)

(2) medial mitral e' >= 9 cm/s

(3) hepatic vein expiratory diastolic reversal ratio >= 0.79
