Malacoplakia (malakoplakia) can rarely be diagnosed in the prostate gland. It is associated with chronic urinary tract infections caused by E. coli.
Malakoplakia:
(1) can mimic prostate cancer on ultrasound and MRI
(2) is associated with elevated serum PSA levels
(3) can co-exist with prostate cancer
Pathology:
(1) fibrohistiocytic proliferation with chronic inflammation and occasional multinucleated giant cells
(2) presence of von Hansemann macrophages with Michaelis-Gutmann bodies (MGBs)
(3) immunostaining with CD164 shows diffuse histiocytic infiltration
