Most cases of fatty change involve macrovesicular deposits of triglycerides.
Features of macrovesicular steatosis:
(1) one or a few large vesicles that displace the hepatocyte nucleus
(2) may be mixed with microvesicular steatosis
Conditions associated with macrovesicular steatosis:
(1) obesity and the metabolic syndrome (nonalcoholic)
(2) alcohol
(3) drugs (acetaminophen, asparaginase, azidothymidine, cisplatin or oxaiplatin, clometacin, corticosteroids, dantrolene, etretinate, floxuridine, flurazepam, gold sodium thiomalate, halothane, ibuprofen, indomethacin, interferon, isoniazid, isotretinoin, methimazole, methotrexate, methyldopa, metoprolol, niacin, nitrofurantoin, rifampin, sulindac, tamoxifen)
The severity of steatosis is based on the percent of hepatocytes affected.
