Description

Macro-TSH is a rare cause of a markedly elevated serum TSH in a patient who may otherwise be euthyroid.


Presentation:

(1) marked elevation of serum TSH

(2) normal serum T3 and T4 concentrations or minor deviations inconsistent with TSH

(3) absence of heterophile antibodies

 

Macro-TSH is a complex of TSH and IgG that is functionally inactive and which accumulates in the blood.

 

The diagnosis can be confirmed by gel filtration chromatography.
