An overdose of loperamide is serious and potentially fatal due to its cardiotoxic effects.
Patient selection: loperamide (Imodium) overdose
Clinical features:
(1) recurrent syncope
(2) cardiac arrest
(3) signs of opioid toxicity
(4) cardiogenic shock
(5) seizures
(6) catatonia
(7) pancreatitis
ECG changes:
(1) QT-interval prolongation
(2) QRS interval widening
(3) ventricular tachycardia including torsades de pointes
(4) ventricular fibrillation
