Vidale et al reported the LARIO (Large ARtery Intracranial Occlusion) Stroke Scale. The authors are from Sant'Anna Hospital and Niguarda Ca' Granda Hospital in Italy.
Patient selection: suspected stroke in a prehospital setting
Parameters:

(2) arm weakness
(3) grip strength
(4) language
(5) neglect

Points
facial palsy
absent (normal)

1
arm weakness
no drift
0
drift OR no effort against gravity OR no movement
1
grip strength
normal
0
reduced or absent
1
language
normal
0
changes, global aphasia, mute
1
neglect
absent
0
extinction to bilateral simultaneous stimulation in one more sensory modality OR own hand unrecognized OR orientation to only one side of the body

total score =
= SUM(points for all of the parameters)
Interpretation:
• minimum score: 0
• maximum score: 5
• The higher the score the more likely a large vessel occlusion.
• A score > 3 had a positive predictive value of 77% and negative predictive value of 100%.
Performance:
• The area under the ROC curve is reported as 0.95.
