The Japanese grade a renal cell carcinoma based on the relative size of tumor cell nuclei. This is outlined in the General Rules of Clinical and Pathologic Studies on Renal Cell Carcinoma.
Parameter: The size of the tumor cell nuclei relative to the nuclei of normal tubular cells.
The nuclei of tumor cells are
Grade
smaller than the nuclei of normal tubular cells

the same size as the nuclei of normal tubular cells
2
larger than the nuclei of normal tubular cells
3
where:
• The area of the nuclei would probably be the most accurate method of comparison.
Grade 3 in the Japanese classification = Grade 3 or 4 in the Fuhrman classification.
