Nugroho et al reported the JAGUAR score as a means to evaluate a patient with ischemic stroke who is not eligible for recanalization therapy. It is intended to be easy, quick and affordable. The authors are from multiple institutions in Japan.
Patient selection: ischemic stroke and ineligible for recanalization therapy
Outcome: severe disability or death at 1 month
Parameters:
(1) Japan coma scale (J)
(2) age in years (A)
(3) random glucose levels (G) in mg/dL
(4) untimely arrival after onset in hours (U)
(5) atrial fibrillation (A)
(6) modified Rankin scale (R) as measure of prearrival dependency status (0 independent, 6 dead)

Interpretation:
• minimum score: 0
• maximum score: 10
• The higher the score the greater the risk of severe disability or death.
